Hi all

I had a scan yesterday and they found a 2nd sac with nothing in it. I had had a massive bleed a week ago where i thought i was miscarrying. My obs has been checking my hcg levels and they are high but not doubling every 2nd day.
I am 6wk2days and found out my progesterone level has dropped from 43.5 to 38. My obs has basically written this pregnancy off and wont even let me try progesterone tabs to help. I have already had 3 mc's :(
Does anyone know whether hcg levels will not do as they should with vanishing twins?? Also does progesterone tabs help rise levels at 6wks gest??

No personal experience with twins but had slow rising HGC (and dr prepared me for mc)but now have happy 14mth old and was coincidentally reading this article just moments ago...

Symptoms of Vanishing Twin Syndrome:

In very early cases of vanishing twin syndrome, the woman might never know that the condition occurred. In other cases, the loss of the twin might be accompanied by some miscarriage symptoms such as vaginal bleeding and hCG levels that rise more slowly than in normally developing twin pregnancies.

Fingers crossed

Progesterone keeps your uterine lining thick. I am on progesterone tablets but my levels dropped from 28 to 9.9. Levels in the first tri should be between 9 and 47 so your progesterone levels are still fantastic. My levels, even supplementing arent that high so I am not sure progesterone is the solution.

Progesterone does not cause your HCG to double. My HCG is very high even though my progesterone levels were low which is why they put me on progesterone. Basically to make sure that my uterine lining remains thick and can support the baby until the placenta starts producing enough progesterone.

Miscarriages can happen for lots of reasons but generally its due to a bad sperm or a bad egg and there is nothing that can be done about it. They sometimes happen due to the corpus luteum being unable to produce enough progesterone in which case your progesterone keeps dropping when it shouldnt and then sends your body a signal to have a period and you miscarry and its only really in those cases that progesterone will help save the pregnancy but in your case, 38 is a great number.

Progesterone fluctuates throughout the day, it doesnt matter if it goes up or down by much, for example, after a large meal, progesterone levels can decrease by 50%. As long as your levels are over 15 or ideally 20 for the first trimester your progesterone is great.

Thanks so much ladies I really appreciate your responses :)
I'm trying to stay positive. On u/s baby is right on target just on paper its not what they are expecting.. I'm not sure how true it is but i read as long as the hcg is steadily rising it might be ok?? Mine is doing it, slowly but steadily ! Although my obs wants it to still double every 48hrs.. So far my levels are 21000. Getting a 2nd opinion tomorrow so fingers crossed!

Once HCG levels are over 10k they dont really double the same way and take much longer to increase.

At 5+3 I was at 9700 at 5+6 I was at around 16k, at 6+6 I was at 46k

I also found out I am having a vanishing twin. At my 7 + 3 scan, there was no longer a heartbeat and then a repeat scan at 7 +4 shows Twin A getting smaller. Once the heart stops beating, or a m/c is happening so early there is nothing they can do :(

HCG can vary so widely. At 6 +1 I was at 41,000 and both twins were still viable at that point.

I know it is hard, we had already planned and talked about having twins. But I am trying to focus on being thankful for being blessed with at least 1 baby.

Thanks ladies :)
Ok went to another obs today and a clear heart beat was seen. Baby is measuring 5wks 6days.. apparently on tuesday the 6wk 1 day was just the size of the sac? This obs said 21000 was a good number as was my progesterone and not to worry!!! OMG never thought i would hear the day!! lol He said there is no point checking hcg levels as he has seen the heart beat. He believes that instead of the vanishing twin it appears i have a clot which could be caused from implantation. He prescribed anti- biotics.
Soooo glad I decided to get a 2nd opinion :) very happy and relieved so i guess its just a waiting game now :)

Vanishing Twin Syndrome Statistics and Risks

Fingers crossed
I know this post is old but how slow were your hcg levels rising? I’m in similar situation. My specialist is a lot more optimistic than I am. Mine slowed to 5.1 days doubling time at just over 5 weeks.

I had a potential twin loss early on, there was one sack. I had a heavy bleed with clots, my hcg was through the roof and it slowed down after the bleed, only to have a scan and repeat bloods, scan showed one heart beat, and my hcg had risen again, there was no exact reason for it other than the doctor taking a guess of it being an early twin loss, I would have just said it was just early pregnancy bleeding, but if that was the case, my hcg wouldn't have done what it did and slowed down, to then speed back up again! My is daughter is 8 now and I cant help but wonder if there was meant to be another one of her
