Filling out Child tax credit form-help-anyone!

I have lost the notes to the child benefit form - in fact I've lost the prepaid envelope too - where can i get another ASAP - I only have a few weeks before Alex is3 months and we start losing money!

Ooh can they only make back payments for up to 3 months? Ruby is 4 months and I'm only just filling form in now! I wrongly assumed we wouldn't get anything.
I would just phone them up and get another sent.
