I've just found out that in not entitled to SMP this time because I haven't made enough contributions as I only work 8 hours a week. I got a letter from my head office to say I need to take my SMP1 form and my original MATB1 form to my local job centre. Do I need to make an appointment? Is there somebody in particular I have to speak to?
 
You just need to ask for the application forms for MA, but you could probably do that over the phone if you look it up on the DWP sites :thumbup:
