MAT B1 form

Hi, does anyone know if you can download this form anywhere please? And if so have you got a link? I have been looking but can't seem to find it.

I am not due to see my MW for ages but I do have my scan next week when I will be 20 weeks exactly so I thought if I could download one, I could take it with me and get one of the hospital MW/doctors to sign it.

I could do with having it before I go on holiday and if I can DL one, it will save me waiting over a month until I see my own MW.

You can't download it hun, the MW can only give it to you after 20 weeks- I got mine at 25 week apt. It's a form she basically hand fills in and signs- but you might be able to ring her up if you need it earlier than ur next apt but u wnt be able to have it before 20 weeks x

I'd be surprised if you could download one to be honest - as it has to be signed off by a doctor or m/w, I assume only they can give them out. Otherwise they could be faked - I think it's the same for doctor's notes. I might be wrong!

Maybe give your m/w a call and ask if you can pop in and get one? I'm sure they'll understand if you're going on holiday, it's pretty important to get them plenty of time ahead of 25 weeks so you have time to book your mat leave off with work.

Go to your GP and ask at the Reception desk they should have it there.
you can fill in your details then leave it to be signed off by your doctor then collect in later no need for appointments.

Or you can make an appointment to explain why you need it.

Like the other girls have already said you cannot download this form as the mw fills it all out, stamps and signs it. They dont usually give this to you until you are around 25/26 weeks. I wouldnt worry to much about getting the form until you are closer to that time hun.

I got my form at my 26th wk mw appointment.

hey hon Im only 17.5 weeks & have my form already! my mw gave it to me last week pre-signed & ready to go for when im 20 weeks!! she's fab ;0)

however i asked on my friends behalf as she had same query as you - mw said she should call her doc/mw & they'll be able to post her a signed form so definitely do that honey!!x

You can't download it and you can only get it from your mw after you are 25 weeks. I won't be getting mine until i'm 26 weeks as that's the only time I could get in with my MW.

Can I ask why you need it before your holiday? I'm assuming you are not going to be away for that long you will be more than 25 weeks when you return or do you go away in a few weeks meaning you would be when you return?x

Just ring and ask for it at 20 weeks, I wouldn't wait till your 25 week appointment because most employers require this before you are 25 weeks, otherwise it could affect your maternity pay.

Yep I would also check with your employer - mine requires my maternity leave to be applied for no later than 25 weeks so I couldn't get my form any later than this - I actually got it at my 21 week appointment. I guess all employers and all practises differ slightly...
