Rise on BTP nappies

How do you know when to increase the rise on a BTP nappy? I'm sure that's a really stupid question but are there any clues that you need to increase the rise?

I knew i needed to increase the rise when i'd given Q a builders bum without realising and his BF poop fizzed out of his nappy and completely filled his vest

some people know when the fit round the legs get too tight.

same way you know when it is time to go up a size in clothes, it just doesnt fit right so you go bigger
