Can someone explain a trigger? and why take it out?

Do you mean an assisted conception trugger shot (IVF/ICSI/IUI)?

You are given a trigger injection (shot) that contains HCG (the pregnancy hormone detected by HPT's)

This triggers ovulation ready for Egg collection or Artificial Insemination.

This HCG transfers to your blood/urine and will make all HPT's show +ve for around 10days after the injection.

The idea is to test it out i.e test each day til you see it disappear totally back to -ve test) that way when you do (hopefully) test positive again you know it is a prgenancy causing it and not the trigget shot.
