The u/s is easy. It is transvaginal, but not scary or anything.
At my clinic, this is how it goes: First you go to the bathroom and fully empty your bladder. Next, the nurse shows you to an ultrasound room and leaves while you get undressed from the waste down like for a normal gyno exam. Before she comes back with the doctor, you get onto the u/s table and cover up with the drape that is provided to you, with your legs up in the stirrups.
When the doctor comes in, he or she will prep the u/s wand by putting a condom on it and then putting lube on it. After that, they insert the u/s wand, or you can as to do it yourself. I always put it in myself, as whenever I've let anyone else do it it's quite a bit more physically (and mentally) uncomfortable for me. If they don't offer to let you do it, you can just ask. They look at the cervix and uterus first, then move the probe further into the vagina to look at each of the ovaries. All in all, it only takes a couple minutes. The wand is very narrow and does not hurt, though if they have to poke around to find your ovaries, it can be a little uncomfortable sometimes.
Afterwards they leave the room while you get up and clean off. (The lube is messy!) Paper towels are provided at my clinic.
It's kind of crazy that you haven't had any monitoring while on Femara. I wasn't even allowed to start on fertility meds until an ultrasound had been done to check out my ovaries and uterus. I always had monitoring on even low dose fertility meds (Clomid, Femara).
