income support help..

i put a claim in for income support but i got told at the meeting yesterday i need to get a MATb1 for proof of my due date where can i get this from?

Doc probably could - phone the surgery and enquire. They should be able to leave it behind the desk for you?

where i live the jobcentre can give you the form and the midwife signs it x

My midwife signed mine but im sure a doctor can. They have to put their midwife number on it x
