hey all we got a letter to tell us we were at the top of the list for IUI treatment
, we were put on the list almost a year ago after everything else failed and found out i only have one tube etc (see my tag for list of problems) anyways we got the letter and appointment etc and with them they have "Welfare of the Child: patient history forms" does anyone had to do these? we're based in Glasgow, i am freaking out about them, we answered no to all but one question which was about health conditions etc, i personally have EDS/Hypermobitility syndrome and my OH has OCD (both minor and controlled) so wouldn't get in the way with future children, i already have a son from another relationship and he has some extra needs but nothing major and we have never had any issues with him where we have needed medical/social service imput etc.... 
anyways should i worry about this what does this mean? it says on the other side where the doctor right "reason for rejection" will/can we be rejected? i feel sooo worried and hopeless
