Maternity leave notice

Hello,

My employer has asked for my letter stating when i will be going on my maternity leave and such. I don't know what i need to write in the letter. Do i need to let them know when i'm returning to work or do i do that later?

Does anyon have any template i could use? Maybe from when you sent yours to your employer!

I'm stuck..

Your doc should be able to write you a letter confirming your due date. The company should have a policy of the amount of time you need to take before baby comes.
Maybe they want the letter from the doctor? I know mine do, for their files.

Mine tells me I should go on leave 6 weeks before baby comes, and that if I want to stay on later I have to get a letter from the doc at the time to confirm that I'm fit. I find 6 weeks a bit much.
 
I didn't have to write a letter i just emailed HR and said when i would like to start maternity leave and that i would like to take the full 39 weeks. They then sent me a letter stating my leave date and return date.

When your 20 weeks you then get a MATB1 form from your Midwife (or you can get from hospital if your seeing then for a scan or whatever before you see your midwife) and that confirms your due date.

Thanks for your advice i think i'll just write them saying when my last day will be and that i intend to take my full 39 weeks paid leave.

39 weeks is great!
I only get 16 here...ah well, i at least get the option to take unpaid leave after that..
