Sick note for ivf

Hi my company have been really good with givin me time off for IVF
They suggested getting a sick note that make my time off official so i can get paid.
Was wondering who i would see about getting one? I f clinic or GP?! Does anybody know? X

i think it would be your GP, my work suggested that i have one too, but was a bit unsure as to what my GP would say, its worth a try though
