Maternity exemption certificate?

Not sure if this is the place to post or not... please move if its not

I lost my NHS card and letter when I went into hospital and now I need a new one because I need to go the dentist and cant afford to pay lol cheap!

Just wondering if it said on the letter they sent how to get a new one if yours is lost?

I would ring your GP and ask them, they'll know what to do. Might need to re-apply.

You can call for a replacement on 0845 6018076, apparently you need your full name, dob, exemption certificate number and possibly nhs number

I have my card.... but how do i use it to get a free dentist appointment? Do i have to say when i book it? Or tick a box on a form or what?? Theres no instructions!!
