I don't have a sample one but I'm more than happy to share mine if it would be of any help?
That would be great if you don't mind?
There you go hun:
I have suffered from M.E. for a number of years and, whilst I am well at the moment, I do get exhausted easily.
In order for me to feel safe, (somewhat) in control and not panic, its important that I feel my wishes and choices are taken seriously and my voice is being heard.
My supporter is my husband Ryan. If, for any reason, I am unable to make a decision, please defer to Ryan.
I am open-minded about the options and choices available during labour and am not categorically ruling anything out. I realise that labour and birth are unpredictable things and that the wishes listed below which are based on my ideal birth experience may well go out of the window.
Labour
I would like to use the pool if it is available.
If the pool is not available, I would like to be free to walk around and change position throughout.
I would like the lights in the room to be kept low and the environment to be kept as quiet as possible.
I would prefer to keep the number of vaginal exams to a minimum.
Monitoring
I do not wish to have continuous foetal monitoring unless it is required by the condition of the baby.
I do not want an internal monitor unless the baby has shown some sign of distress.
Anaesthesia/Pain Medication
I would like to use the pool if it is available.
I am open minded about my pain relief options; please suggest anything you think would be appropriate.
If I request an epidural, please take me seriously and provide one as soon as you are able. I wont ask for one unless I feel I really need one. I realise this will mean that I am unable to use the pool and will need to have continuous foetal monitoring.
Caesarean
If a Caesarean delivery is indicated, I would like to be fully informed and to participate in the decision-making process.
I would like Ryan present at all times if the baby requires a Caesarean delivery.
If the baby is not in distress, the baby should be given to Ryan immediately after birth.
Episiotomy
Please only give me an episiotomy if, in your professional opinion, a bad tear is likely without one. Or if the baby is in distress and needs to be born quickly.
I would like a local anaesthetic to repair a tear or an episiotomy.
Delivery
I would appreciate guidance on when to push and when to stop pushing so the perineum can stretch.
If I am labouring in the pool, I would like to give birth there if possible.
I would like to be allowed to choose the position in which I give birth.
Even if I am fully dilated, and assuming the baby is not in distress, I would like to try to wait until I feel the urge to push before beginning the pushing phase.
I would appreciate having the room lights turned low and the room as quiet as possible for the delivery.
I would like as few people as possible present during delivery.
Providing the baby is not in distress, I would like to have skin to skin contact as soon as possible after delivery.
Please wait for the cord to stop pulsating before cutting it unless there is a reason to cut it immediately.
If my baby has to be taken away immediately after delivery, I wish to be keep informed of what is happening at all times and, if possible, Ryan will accompany the baby at all times.
Post Delivery
I would like a private room, if available.
Unless required for health reasons, I do not wish to be separated from my baby.
If possible, I wish to have a managed third stage in order to deliver the placenta quickly.
I would like the baby to receive a dose of Vitamin K.
Breastfeeding
I plan to breastfeed the baby and would like to begin nursing shortly after birth. Help/guidance would be appreciated.
Unless medically necessary, I do not wish to have any bottles given to the baby.
