Smp/ma forms?

OK ive been off sick for 3 weeks from work but i only work 8 hrs so i wont get smp from work, when do they need to give me the correct form to state this? as ive asked them like 6 weeks to sort stuff i need and they are like ignoring me, i know ill need MA and im leaving work asap (29weeks) but is there anyting i can do to get them moving as i need to send the MA form off soon too? even that form is confussing! x

your employer is obliged to fill in form SMP1

When you get your MATB1 from the midwife (around 25 weeks) you hand this into your employer. They will return it with SMP1. You need both to be able to claim MA. You will also need 13 weeks worth of payslips showing your highest wages from the past 66 weeks. When you download the forms you get a chart to calculate that period.

I've literally (last friday) done mine after getting my forms back.
